
Advance Care Planning – 5 Part Series
Join us for a 5 part series on Advance Care Planning. Please note that it is a closed group, which means that additional attendees will not be able to join the series for this session after the first night.
Advance Care Planning is a process of reflection and communication. It is a time for you to reflect on your values and wishes and let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself.
In Ontario, advance care planning (ACP) is:
- Confirming your substitute decision-maker(s) (SDM)
- Communicating your wishes, values and beliefs about care to help your SDM(s) make health and personal care decisions for you if you become mentally incapable of doing so for yourself.
Join us for this interactive 5 part series. This workshop includes a weekly workbook instructional video sent on the Monday before the interactive discussion following Thursday. Each video will be made available by Monday at 4 pm that talks about each step. On the following Thursday, we will have an interactive conversation led by Sandra Andreychuk, Health Care Ethicist.
